Survived by wife, Myrena; children, Sarah and husband Brian White of Omaha, and Bryan Van Slyke of Papillion; mother, Noreen Van Slyke of Indianapolis, IN; sister, Elaine and husband Rob Williams, Lebanon, IN; brothers, Steve and wife Teresa Van Slyke of Indianapolis, IN, and Rick and wife Windy Van Slyke of Chicago, IL; several nieces and nephews. Family will Receive friends Monday, June 9, 4-8pm at Bellevue Memorial Chapel, with a Vigil at 7pm. MASS OF CHRISTIAN BURIAL: Tuesday, June 10, at 10am at St. Matthew’s Catholic Church, 12210 So. 36th St., Bellevue. Interment: St. John’s Cemetery.
