Philip Calek October 31, 1957 ~ October 30, 2015 Preceded in death by parents Joseph and Evelyn; brother Joseph Calek Lt. Col USA. Survived by sister Terry Calek (Lee Polikov), brother Rodney Calek (Debbie); nephews Brian; Chad and niece Katie. The family thanks the Oncology Department at Creighton Medical Center with deepest appreciation to the oncology nurses who assisted us in so many ways.
