 Survived by daughters: Suzanne M. Hazuka, Julie L. Hazuka, Cheri L. West, and Heidi M. Newman (Jeff); five grandchildren; four great-grandchildren; siblings: Daniel Hazuka, Carol Gonzalez (Mike), and Deb Molek (Dave); nieces and nephews; other relatives and friends.

VISITATION following CDC guidelines: Tuesday, September 22^nd from 10am to 11am, at the West Center Chapel followed by FUNERAL SERVICE at 11am.  Interment, Evergreen Memorial Park with military honors by Benson VFW Post 2503. Memorials are suggested to the Leukemia and Lymphoma Society.
