Preceded in death by parents, Ivan and Frances (Sedlak) Polak; sisters, Georgia Polak Siragusa (Phil); and Dolores Polak Cimino (Jim); brothers, Richard Polak (Rita); and Leo Polak; and nephew, Michael Siragusa.
Survived by Notre Dame Sisters; sister, Rosemarie (Polak) Rosenberger (Dennis); sister-in-law, Liz Polak; nieces and nephews.
VIGIL: Tuesday, April 23rd, 7 p.m., at Notre Dame Chapel, 3501 State St.
MASS OF CHRISTIAN BURIAL: Wednesday, April 24th, 10 a.m., at Notre Dame Chapel.
Interment, Calvary Cemetery.
Memorials may be directed to Notre Dame Sisters of Omaha.

Rest in Peace, Sister Joan. It was pleasure to have known you from your days at Assumption Grade School in South Omaha many years ago.

Through the love of God, I pray that he sends you sisters of Notre Dame, family and friends the comfort that only he can convey.
So glad I was able to see you at the Celebration dinner, 2 weeks ago. You gave me one of the greatest gifts by teaching me how to read, in 1st grade at Assumption School. Thank you for that skill an everything else you have done for me. May you enjoy your heavenly rewards. You have helped so many people with your goodness.
So grateful to have known you and your gifts to the nutrition center, Casa Carolina and the sewing center Academia de Madre Theresa Gerhardinger in Sulaco Yoro, Honduras. We had some memorable times together.
